The company got a lot of feedback regarding some of the pages being damaged (mainly water spots) in addition to the PlayStation 4 dynamic theme codes not being able to redeem on the Sony Entertainment Store. They are suggesting that customers who are having issues with the product should contact their Email support to get replacements:

The Bloodborne Collector’s Edition Strategy Guide had a troublesome launch as it ended up being delay by nearly a month after the videogame was made available. From Software made some changes to Bloodborne via a first-day patch, forcing the book publisher to make a last-minute decision to delay the guide in order to make it up to date in addition to adding more content.
